Chapter transcript
- 00:42:46 and then financial conflicts of interest like everywhere.
- 00:42:50 Beginning of this year,
- 00:42:52 the New England Journal published an absolutely horrific article where they
- 00:42:57 concluded that only clinical trial data has any validity and anecdotal data or
- 00:43:03 empirical real-world evidence is not to be used to set medical standard of care.
- 00:43:09 The clinical trials is a big part of the content of the New England Journal.
- 00:43:12 Jeff Drazen,
- 00:43:13 when he became the editor,
- 00:43:16 said that he wanted to move the journal toward publishing more basic science and more clinical trials.
- 00:43:22 He thought there was too heavy an emphasis on epidemiology,
- 00:43:26 observational research.
- 00:43:27 I trust the New England Journal of Medicine.
- 00:43:29 That's my journal.
- 00:43:31 It's like it's corrupt.
- 00:43:33 That's what the insiders have said.
- 00:43:35 The whole thing is a pile of crap.
- 00:43:37 A journal that has to pay its editors and its editorial staff and so on,
- 00:43:42 that can't make all of its money on article submission fees,
- 00:43:45 has to raise money and does it by advertising and becomes addicted to the advertising.
- 00:43:50 And once that happens,
- 00:43:51 which happens fairly quickly,
- 00:43:52 the publisher of the journal has to kowtow to the interests of pharma and those interests are very clearly expressed and feed down into the editors themselves.
- 00:44:01 And Horton at Lancet and Marsha Angel at New England
- 00:44:03 Journal all talked about this in their books and comments
- 00:44:06 about the effects of pharma on medical publishing.
- 00:44:09 The randomized placebo-controlled trial is in fact the gold standard.
- 00:44:13 We have a whole vast medical literature that that was not the standard.
- 00:44:17 So why is it suddenly the standard now?
- 00:44:19 All of the evidence needs to go into this discussion,
- 00:44:22 not just a shibboleth of randomized controlled trials.
- 00:44:25 The drug companies oftentimes do the clinical trial that doesn't answer the clinical question.
- 00:44:31 The physician will want to know,
- 00:44:33 "Is your drug better than the drug I'm using now?" But in order to get FDA approval,
- 00:44:38 the company will do a placebo-controlled trial,
- 00:44:41 assuming that it's ethical to do that.
- 00:44:43 And so they don't answer the question the doctor really wants them to answer.
- 00:44:47 Questions like,
- 00:44:48 "Despite a 5.9 billion dollar budget,
- 00:44:51 why isn't Dr.
- 00:44:52 Fauci able to provide us with a study that shows whether or not
- 00:44:55 hydroxychloroquine works early in tandem with zinc and Z-Pak?"
- 00:44:59 Practicing physicians are going to rely on review
- 00:45:02 articles in the New England Journal of Medicine,
- 00:45:05 not realizing that they're actually written by the company.
- 00:45:10 The clinical trial system is maybe broken,
- 00:45:14 is too strong a term,
- 00:45:15 but it's definitely underperforming.
- 00:45:19 All the negative papers that have appeared showing hydroxychloroquine doesn't work demonstrate profound ignorance in the pathology of COVID-19 as a disease.
- 00:45:29 Relying on a so called theoretical randomized controlled trial is a red herring because that is not the way the FDA works,
- 00:45:36 it is not the way scientists work,
- 00:45:37 it is not the way anybody works in real world.
- 00:45:40 Well of course hydroxychloroquine,
- 00:45:42 as we've said before,
- 00:45:43 is very cheap.
- 00:45:44 Big Pharma does not make its money on cheap generics,
- 00:45:47 but they do give a lot of money to NIH,
